First Visit

Your initial appointment  will consist of an examination and review of your treatment options. This appointment is a 2 hour commitment reserved just for you.  We can split the 2 hour initial appointment into 2 1 hour sessions with your first appointment being with Dr. Shackelton.  We will take necessary images, Intra-Oral photos, examine your gum and bone health, perform an oral cancer screening and a full examination for tooth decay.  You will have time to discuss fully your concerns and questions with Dr. Shackelton.

A complex medical history or treatment plan will require a medical consultation and in some cases, a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable. A records release form is required for us to request these for you.
  • A list of medications and supplements you are presently taking.
  • If you have dental insurance, we collect the information when you schedule to verify eligibility and benefits for you as a courtesy and file all claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your prior dentist  has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they will be taken at our facility.  A records release form will be necessary to submit to your prior Dentist for these records. We are able to assist with this transfer and request.
